The Enrichment Centre for Mental Health is looking for a Case Worker to manage a small but intensive case load to join our team.
If you thrive in a busy environment with dedicated coworkers, and have these further qualifications:
- 4 years experience working with seniors and vulnerable individuals
- Seasoned systems navigator – especially in primary care
- Minimum human services diploma and registered with OCSWSSW
- Must have their own vehicle
- CPIC/CPR
- Strong case management including documentation and scheduling
